Lazy Kid or Depressed? When do you worry?

I have been a single dad for most of my daughters' life. She is 18 now and for the past 5 years, it has been an unbelievably hard road. After the sudden loss of her grandmother (my mother back in 2009) she started cutting herself. I have, on several occasions, had to bring her to the hospital for either stitches or for suicidal ideation. It's been hell.
